You can now refer patients to our office using a secure, HIPAA-compliant online form powered by JotForm—no fax machine required. Simply submit your patient’s information and exam notes through the form, and our team will safely receive the details.

Once submitted, our staff will promptly contact the patient and schedule the appropriate appointment.
